C.O.A.C.H. Award

How to apply:

All applicants must apply directly to Coach4drake@gmail.com by no later than May 6th. To be considered for award, applicants must:

  1. Letter from High School Guidance Counselor
  2. College Letter of Acceptance (Official acceptance letter from a 4-year college or university, including name, date, and school)
  3. How did you hear about the COACH4Drake Award?
  4. Current Picture/Identification
  5. What hardships are you facing: (i.e. financial, emotional, academic or family challenges etc.)
  6. Write a Research Article on Epilepsy (1,000 words, single-spaced).
  7. If you win this scholarship, what is your future plan of involvement with the foundation?
  8. If chosen as a finalist, you may be required to interview via ZOOM. Do you have access to this platform?
  9. Provide the best method of contact (email and contact number).

How to volunteer:

Support our Annual 5k Walk/Run each year (always held the weekend before Mother’s Day). This year’s 5K is Saturday, May 6, 2023.
